Home Care is a growing industry and a valuable part of the Health Care continuum. It allows people to leave hospitals and nursing homes earlier than before by bringing the necessary care to clients in the comfort of their own homes. Statistics show that people are happier and recover quicker in their own familiar surroundings. We strive to help people regain health and mobility to live a healthy and independent lifestyle. Our highly trained staff is dedicated to improving quality of life at each home visit. We work with clients and their physicians to create a customized, personal plan of care to best fit the individual needs and circumstance of each person in our care. We offer the following services in support of that goal:
Homemaking includes light housekeeping, laundry, meal preparation, and grocery shopping. Personal care includes assisting the client to bathe, dress, and attend to other personal hygiene of mobility needs as required.
Certified Nurse Aides are licensed and are able to provide a higher skill level of personal care needs, for those with greater physical limitations. This could also include bathing, skilled transfers, skin care, ostomy care, checking vital signs, assisting with range of motion exercises or ambulation exercises, incontinence care, and feeding. They are supervised by a Registered Nurse or Licensed Physical Therapist.
Registered nurses and therapists are available to assess, evaluate, monitor, and provide specialized skilled services. These services are coordinated with the client’s physician.
Our CHHC team not only includes our expert nurses, therapists, and aides, but also includes those you hear but may not see. Our "behind the scenes" staff work hard for you to help coordinate all services included in our clients' plans of care to provide the best overall care to all our clients.
