CHHC provides the highest quality home health services to clients in the comfort of their own homes to help them live a healthy and independent lifestyle. Our skilled nursing, therapy, personal care and homemaking professionals are dedicated to improving quality of life at each home visit by working with clients' physicians to coordinate a personal plan of care to best fit our clients' needs.  We offer the following services in support of that goal:

Homemaking and Personal Care

Homemaking includes light housekeeping, laundry, meal preparation, and grocery shopping. Personal care includes assisting the client to bathe and dress.

Certified Nurse Aides

Certified Nurse Aides are licensed and can do home-making, personal care, and perform limited skilled services such as taking vital signs and assisting the client to do range of motion exercises when set-up by a Physical Therapist Occupational Therapist.

Registered Nurses and Therapists

Registered nurses and therapists are available to help monitor and provide specialized skilled services. These services are coordinated with the client’s physician

Home Care Team

Our CHHC team includes our expert nursing staff, physical therapists, occupational therapists, speech therapists, medical social workers, certified nursing assistants and personal care workers.  Our staff coordinates all services included in our clients' plans of care to provide the best overall care to all our clients.
