Cardiac and Pulmonary Care

Skilled nursing staff at CHHC offers expert care in monitoring cardiac/pulmonary disease symptoms and help in disease management for these problems. Individualized plans of care are coordinated with client physicians to address their symptoms. Nurses will monitor blood pressure, blood oxygen saturation levels, pulse, weight and temperature, and communicate this information to the client's physician for his review. 

We have on-call staff available to answer your questions. As well as caring for chronic disease, our nursing and therapy staff are trained and experienced in caring for clients post-MI (heart attack) and post-cardiac surgeries, including cardiac bypass surgery. We provide a team approach to care that ensures clients are back to their maximum capacity as quickly as possible, while learning the lifestyle changes necessary to enjoy life to the fullest.
